Provide a Birth Certificate (must be certified with a raised seal). The State of Michigan requires the School District to inform authorities if an original, certified birth certificate has not been produced within the requested time frame. We will make a copy of it at our office when you bring it.
Provide Immunization Records
Parent/Guardian Photo ID (Driver's License or Passport)
Provide Two Proofs of residence:
Homeowners -If you own or are purchasing a home in the district, please provide:
Current utility bill in the name and address of the person enrolling the student (gas, electric, water, or cable bill only) which matches the service address
One of the following: Current Property Tax Bill with name and address; or Purchase Agreement (if closing occurred within the last six months).
Tenant – If you are renting a home in the district, please provide:
Current utility bill in the name and address of the person enrolling the student (gas, electric, water, or cable bill only) which matches the service address
Special Residency – If the parent/child lives with another family within Lakeshore school district, we require a residency affidavit completed and notarized by our Central Office so both parties need to come in to have their signatures notarized along with the ID of the parent and residency information from the person the family is living with.
